Carya Medical LLC, operating under the direction of Renata Angelini, MD ("Carya Medical," "we," "us," or "our"), located in Port St. Lucie, Florida, values your privacy and is committed to protecting the personal and health information of our patients and website visitors. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit our website (the "Site"), contact our office, or interact with our SMS text messaging service.
This Privacy Policy applies to information collected through the Site, by telephone, by email, in our office, and via SMS/text messaging. It does not apply to information collected by third-party websites linked from our Site.
Privacy Policy -Information we collect
a. Information You Provide Directly When you use our Site, contact us, or request an appointment, we may collect: personal identifying information (name, phone number, email address, mailing address, date of birth); health-related information you voluntarily share (which may constitute Protected Health Information, or "PHI," under HIPAA); and any other information you choose to submit through a contact form or appointment request.
Please do not include sensitive health information (such as details about a diagnosis or treatment) in the Site's contact form. Contact form submissions are not a secure or HIPAA-compliant method of communication. For health-related matters, please call our office at (754) 212-4625.
b. Information Collected Automatically When you visit our Site, we (and any third-party analytics providers we use) may automatically collect certain technical information, such as IP address, browser type, device type, pages visited, and the date and time of your visit, typically through cookies and similar technologies. c. SMS/Text Messaging Information Our office also offers an SMS text messaging service for patients. Enrollment in this service takes place in our office through a signed consent form — it is not something you can opt in to through this website. Once enrolled, we collect the information described in the SMS Terms and Conditions section, including your mobile number, carrier, and the content of messages exchanged.
Privacy Policy - How We Use Your Information
Schedule, confirm, and remind you of appointments; communicate regarding treatment, billing, or administrative matters; respond to inquiries; send health tips or updates (via SMS, with consent); improve the Site; comply with legal obligations.
Site not directed to children under 13; no knowing collection outside the context of providing pediatric/family care to patients.
Our Site may contain links to third-party websites or services, such as social media pages or map/directions services, that are not owned or controlled by Carya Medical LLC. We are not responsible for the privacy practices of those third-party sites and encourage you to review their privacy policies separately. If we add features such as a patient portal or online payments in the future, this Policy will be updated accordingly.
Updates posted on the website with a new effective date; material changes also communicated via SMS where appropriate.

Changes to This Policy
SMS opt-out by revoking signed consent and notifying office staff (EMR appointment reminders continue separately). Right to access/copy medical records per HIPAA Notice and Fla. Stat. § 456.057. Other communication channels continue even after SMS opt-out.
Your Choices & Rights
Medical records retained per Fla. Stat. § 456.057 — minimum five years from last patient contact, or until age 21 for minors, whichever is longer.
Data Retention
Reasonable administrative, technical, and physical safeguards are used, consistent with HIPAA. No transmission method (internet or SMS) is fully secure; avoid sending sensitive data (e.g., SSN) via SMS or unencrypted forms. In the event of a breach of unsecured PHI, notification follows HIPAA and, for Florida residents, the Florida Information Protection Act (FIPA) — affected individuals notified without unreasonable delay, and the Florida AG notified within 30 days if 500+ FL residents affected.
Protection of Your Information
We do not share or sell information for marketing purposes. We may disclose information to comply with legal obligations, with your consent, or to service providers performing work on our behalf (bound by contract to protect it), or as permitted under HIPAA for treatment, payment, and healthcare operations. Text messaging originator opt-in data and consent is never shared with third parties.
Disclosure of Information
Our Site is built and hosted on Wix Studio. Wix automatically places essential cookies that are necessary for the Site to function and remain secure, such as cookies that maintain your browsing session and protect against fraud. Our Site also uses Google Analytics, which places additional cookies to help us understand how visitors use the Site, such as which pages are viewed and for how long. We use a cookie consent banner to inform visitors about these cookies and to collect consent where required by law. That Notice controls in the event of any conflict with this Policy regarding PHI.
